Pelvic Floor Dysfunction: Symptoms, Causes, and Treatment

3

Pelvic floor dysfunction occurs when the muscles and supporting tissues of the pelvic floor do not work properly. These muscles help support pelvic organs and contribute to bladder, bowel, and sexual function.

The pelvic floor needs to contract and relax appropriately. Problems can occur when these muscles are too weak, too tight, poorly coordinated, or otherwise impaired.

Symptoms of pelvic floor dysfunction can vary. Some people experience urinary leakage, difficulty starting or controlling urination, frequent urination, constipation, difficulty emptying the bowel, pelvic pressure, or pain.

Pelvic pain may occur in the lower abdomen, hips, lower back, or genital region. Some people may also experience discomfort during sexual activity.

Several factors can contribute to pelvic floor problems. Pregnancy and childbirth, pelvic surgery, chronic constipation, repeated straining, certain injuries, aging, and changes in muscle function can all play a role.

Diagnosis generally begins with a healthcare professional taking a detailed history and assessing symptoms. A physical examination may be used to evaluate pelvic floor muscle function.

Treatment depends on the specific problem. Pelvic floor physical therapy is one commonly used approach. A trained therapist may teach exercises and techniques designed to improve strength, flexibility, relaxation, coordination, or control.

Importantly, pelvic floor exercises are not appropriate in exactly the same way for everyone. Someone with overly tight pelvic floor muscles may need relaxation and coordination techniques rather than simply performing more strengthening exercises.

Lifestyle strategies may also be recommended. Managing constipation, maintaining appropriate hydration, addressing bladder habits, and following professional exercise guidance may support improvement.

People experiencing persistent pelvic pain, urinary or bowel problems, or sexual discomfort should seek medical advice rather than assuming these symptoms are simply part of aging or childbirth recovery.

Understanding pelvic floor dysfunction can help people recognize that pelvic health problems are common and may be treatable. Professional assessment can help identify the underlying issue and guide an individualized treatment plan.
