Stiff Mid-Back from Desk Work? Why Movement Matters (Plus 3 Simple Exercises)

If you spend your workday at a computer, you have probably felt it: a dull ache between the shoulder blades, a stiff upper back, and the urge to stretch every afternoon. Mid-back, or thoracic, stiffness is one of the most common complaints we see in office workers at our Port Credit clinic, and in most cases, it is very manageable.

Why Does Your Mid-Back Get Stiff at a Desk?

Your thoracic spine is designed to move: to rotate, extend, and bend. When you sit in a slumped position for hours, several things happen:

  • Sustained load on normal tissues: ligaments and joints held at the end of their range for long periods start sending discomfort signals

  • Reduced movement: joints that do not move regularly become stiff, and surrounding muscles lose endurance

Postural Syndrome vs Postural Dysfunction (McKenzie Method)

The McKenzie Method helps us classify mid-back and posture-related pain so treatment is specific. Our physiotherapist, Simoni Bhagat, uses this same framework when assessing back and neck pain at our clinic.

Postural Syndrome

Pain comes from normal, healthy tissue being held in a sustained end-range position, such as slouching. Think of bending your finger back and holding it: at first it is fine, but after a while it aches. Nothing is damaged, and the pain goes away as soon as you change position.

  • Pain is intermittent and only appears after sustained sitting or standing

  • Movement is full and pain-free

  • Pain eases quickly when you move or correct your posture

Postural Dysfunction

Over time, prolonged poor posture can lead to tissues adapting and shortening. Pain is now felt at the end of available movement, and range of motion is reduced.

  • Pain is consistent at the end of range, like when you try to fully straighten or rotate

  • Movement is limited in one or more directions

A McKenzie-trained physiotherapist can determine which pattern you have, and whether something else, like a joint derangement, is involved.

3 Simple Seated Exercises for Mid-Back Stiffness

Try these every 30 to 60 minutes of sitting. Move slowly and within a comfortable range. Mild stretching is normal, but sharp or worsening pain is not.

1. Seated Cat-Cow

Sit tall near the front of your chair with feet flat and hands resting on your knees. Breathe in as you gently arch your back, lifting your chest and looking slightly upward. Breathe out as you round your back, tucking your chin and drawing your belly in. Repeat 8 to 10 times.

2. Seated Thoracic Extension

Sit back in a chair with a backrest that reaches around the middle of your shoulder blades. Clasp your hands gently behind your head to support your neck. Lean your upper back over the top of the backrest, opening your chest toward the ceiling, and then return to upright. Keep your lower back supported and avoid pushing into pain. Repeat 8 to 10 times.

3. Seated Forward Bend

Sit with feet wider than hip-width apart. Slowly bend forward, letting your chest move toward your knees and your arms hang toward the floor. Pause for a breath or two, then roll back up slowly, stacking one vertebra at a time. Repeat 5 to 8 times. If you have a disc-related back condition or sciatica, check with your physiotherapist before doing this one.

Simple Desk Habits That Help

  • Set your screen at eye level, about an arm's length away

  • Keep your elbows close to your body with shoulders relaxed

  • Use a small lumbar roll or cushion to support your lower back

When to See a Physiotherapist

If stiffness is lasting more than a couple of weeks, keeps returning, causes headaches, or radiates around your ribs or into your arms, a physiotherapy assessment can identify the cause and give you a targeted plan. We can also provide ergonomic advice specific to your workstation.
