In orthopedic surgery, for example, various agents are used to limit blood loss from cancellous bone, while in vascular surgery, fibrin sealants are often preferred to close tissue defects.

The Role of rFVIIa as a Systemic Hemostatic Agent

Recombinant factor VIIa (rFVIIa) is best understood as a potent systemic hemostatic agent within the larger realm of Bleeding disorder therapy. Unlike local agents that act at the site of application, rFVIIa is administered intravenously to correct a dysfunctional coagulation pathway systemically. Its primary mechanism is to enhance the thrombin generation on activated platelets at the site of injury, leading to a robust and localized "thrombin burst" that stabilizes the clot. This makes it exceptionally effective in the management of severe bleeding in patients with hemophilia and inhibitors.
